dc.description.abstract | Determinar la efectividad de la escala ecográfica TI-RADS para el diagnóstico de Cáncer de Tiroides en el Servicio de Radiodiagnóstico del Instituto Regional de Enfermedades Neoplásicas “Dr. Luis Pinillos Ganoza”-Trujillo, durante el período 2017- 2019. Material y métodos: Este estudio corresponde a un estudio de tipo Transversal de Pruebas diagnósticas, se incluyeron 89 pacientes con Cáncer de Tiroides, del Instituto Regional de Enfermedades Neoplásicas “Dr. Luis Pinillos Ganoza”-Trujillo. Resultados: Se incluyó a 89 pacientes que cumplieron con los criterios de selección observamos que el sexo femenino fue el que predomino en relacion al sexo masculino con un total de 69 casos (87%); con respecto a la edad 46 pacientes (85%) tenian entre 41 a 70 años, y 23 pacientes (29%) tenian un rango de edad entre 19 a 40 años, ademas; se encontraron 40 nódulos tiroideos con características de malignidad media entre 2 a 4 cm, representando el (51%), y 28 nodulos (35%) midian mas de 4 cm, se reportaron ecograficamente que el lobulo derecho mostro presentar la mayor cantidad de nodulos 42 (53%) con respecto al lobulo izquierdo donde se reportaron 38 nodulos (48%); y 41 pacientes (52%) presentaron clínica compresiva al momento del diagnóstico. El Carcinoma Papilar fue el más frecuente, con 61 casos, siendo el (77%) Los resultados del desempeño de la escala TI-RADS mostró una sensibilidad de 97%, una especificidad del 80%, valor de predicción positivo (VPP+) 97%, valor de predicción negativo (VPP-) 80%, la razón de verosimilitud positivo (LR+) de 4.87, la razón de verosimilitud negativo (LR-) de 0.03, con una probabilidad preprueba de 89% y una precisión del 80%. Se estableció asociación significativa entre las variables Escala Ecográfica TIRADS y el Cáncer de Tiroides, esta relación χ²= 0.459 ̧p-value es < 0.00001. Conclusiones: La Escala Ecográfica TIRADS, tiene un buen rendimiento diagnóstico y constituye una herramienta adecuada en el manejo inicial del Cáncer de Tiroides, siendo considerada como una prueba de screening o tamizaje. | es_PE |
dc.description.abstract | To determine the effectiveness of the TI-RADS ultrasound scale for the diagnosis of Thyroid Cancer in the Radiodiagnosis Service of the Regional Institute of Neoplastic Diseases ““Dr. Luis Pinillos Ganoza”-Trujillo, during the period 2017-2019. Material and methods: This study corresponds to a cross-sectional study of diagnostic tests, 89 patients with Thyroid Cancer were included, from the Regional Institute of Neoplastic Diseases ““Dr. Luis Pinillos Ganoza”-Trujillo. Results: 89 patients who met the selection criteria were shown, we observed that the female sex was the one that predominated in relation to the male sex with a total of 69 cases (87%); Regarding age, 46 patients (85%) are between 41 and 70 years old, and 23 patients (29%) have an age range between 19 and 40 years, in addition; 40 thyroid nodules were found with characteristics of medium malignancy between 2 to 4 cm, representing (51%), and 28 nodules (35%) measured more than 4 cm, it was reported by ultrasound that the right lobe showed the highest number of nodules. 42 (53%) with respect to the left lobe where 38 nodules (48%) were reported; and 41 patients (52%) presented compression symptoms at the time of diagnosis. Papillary carcinoma was the most frequent, with 61 cases (77%). The results of the performance of the TI-RADS scale showed a sensitivity of 97%, a specificity of 80%, positive predictive value (PPV+) 97%, negative predictive value (PPV-) 80%, positive likelihood ratio (LR+) of 4.87, negative likelihood ratio (LR-) 0.03, with a pretest probability of 89% and an accuracy of 80%. A significant association was established between the TIRADS Ultrasound Scale variables and Thyroid Cancer, this relationship χ²= 0.459, p-value is < 0.00001. Conclusions: The TI-RADS Ultrasound Scale has good diagnostic performance and is an adequate tool in the initial management of thyroid cancer, being considered as a screening test. | en_US |
