Hallazgos tomográficos como predictores de mortalidad en hemorragia intracerebral espontanea en el hospital Belén de Trujillo

Determinar si la localización del hematoma, el volumen del hematoma, la
hemorragia intraventricular y la desviación de la línea media son predictores
imagenológicos de mortalidad en pacientes con hemorragia intracerebral espontánea
atendidos en el Servicio de Neurología del Hospital Belén de Trujillo durante el periodo
2012 – 2015.
Material y métodos: Estudio de pruebas diagnósticas, retrospectivo, observacional, en
102 pacientes con hemorragia intracerebral espontanea. Se calculó la sensibilidad,
especificidad, valor predictivo positivo y negativo. Se aplicó el test de chi cuadrado y
área bajo la curva.
Resultados: El hematoma infratentorial es predictor de mortalidad con una sensibilidad
de 79%, especificidad 87%, VPP 66% y VPN 93%. El volumen del hematoma ≥ 60 cc
es predictor de mortalidad con una sensibilidad de 88%, sensibilidad 90%, VPP 72% y
VPN 96%. La hemorragia intraventricular es predictor de mortalidad con una
sensibilidad de 83%, especificidad 85%, VPP 63% y VPN 94%. La desviación de la
línea media > 10 mm es predictor de mortalidad con una sensibilidad de 71%,
especificidad 82%, VPP 57% y VPN 90%; estableciendo que volúmenes superiores a
60 cc tienen mayor pronóstico de mortalidad en hemorragia intracerebral espontánea. El
hematoma infratentorial, el volumen ≥ 60 cc; la hemorragia intraventricular y la
desviación de la línea media son predictores tomográficos de mortalidad en hemorragia
intracerebral espontanea en el análisis multivariado. El área bajo la curva de los 4
predictores tomográficos integrados para pronóstico de mortalidad en hemorragia
intracerebral espontanea fue de 87%.
Conclusiones: La localización del hematoma, el volumen del hematoma, la hemorragia
intraventricular y la desviación de la línea media son predictores imagenológicos
de mortalidad en pacientes con hemorragia intracerebral espontánea en el Hospital
Belén de Trujillo. Determine whether the location of hematoma, hematoma volume,
intraventricular hemorrhage and deviation from the midline are imaging predictors of
mortality in patients with spontaneous intracerebral hemorrhage treated at the
Neurology Department of Trujillo Belen Hospital during the period 2012-2015.
Material and methods: Diagnostic study, retrospective, observational, in 102 patients
with spontaneous intracerebral hemorrhage tests. Sensitivity, specificity, positive and
negative predictive value was calculated. Chi square test and area under the curve was
applied.
Results: The infratentorial hematoma is predictor of mortality with a sensitivity of 79
%, specificity 87%, VPP 66% y VPN 93%, The hematoma volume ≥ 60 cc is predictor
of mortality with a sensitivity of 88%, specificity 90%, VPP 72% y VPN 96%. The
intraventricular hemorrhage is predictor of mortality with a sensitivity of 83%,
specificity 85%, VPP 63% y VPN 94%. The deviation of the line > 10 mm is predictor
of mortality with a sensitivity of 71%, specificity 82%, VPP 57% y VPN 90%;
establishing that over 60 cc volumes increase the mortality prognostic in spontaneous
intracerebral hemorrhage. The infratentorial hematoma volume > 60 cc; intraventricular
hemorrhage and deviation from the midline are tomographic predictors of mortality in
spontaneous cerebral hemorrhage in the multivariate analysis. The area under the curve
of the 4 integrated tomographic predictors for prognosis of spontaneous intracerebral
hemorrhage mortality was 87 %.
Conclusions: The location of hematoma, hematoma volume, intraventricular
hemorrhage and deviation from the midline are imaging predictors of mortality in
patients with spontaneous intracerebral hemorrhage in the Belen Hospital of Trujillo.
