Evaluar la validez del NEXUS Chest como instrumento de
decisión para radiografías de tórax selectiva en pacientes con trauma torácico
cerrado en el Hospital Regional Docente de Trujillo durante el periodo de Enero
del 2010 a Octubre del 2014.
MATERIAL Y MÉTODO: Se realizó un estudio observacional, analítico, de
pruebas diagnósticas, que evaluó 120 pacientes con trauma torácico cerrado,
distribuidos en los grupos I y II, con 96 y 24 pacientes con hallazgos radiográficos
positivos y negativos respectivamente.
RESULTADOS: La edad promedio global fue 45,41 ± 18,57 años y el 71,67%
de toda la muestra estudiada correspondió al sexo masculino. El promedio del
NEXUS CHEST en el grupo I y II fueron 2,16 ± 0,69 y 1,46 ± 0,93
respectivamente. En lo que respecta a las causas del trauma, en ambos grupos
predominaron los accidentes de tránsito, seguidos por las caídas de altura; y el
tipo de lesiones que predominaron fueron los hemotórax, seguidos por los
neumotórax y fracturas costales múltiples. Al asociar las lesiones intratorácicas
y el NEXUS CHEST, se observó que 83,62% de los pacientes con un NEXUS
CHEST positivo tuvieron lesiones intratorácicas y 25% de los pacientes que
tuvieron un NEXUS CHEST negativo tuvieron lesiones intratorácicas (p < 0,01);
con respecto al rendimiento del NEXUS CHEST, para predecir lesiones
intratorácicas, a un punto de corte ≥ 1, la sensibilidad, especificidad, VPP y VPN
fueron 98,98%, 13,64%, 83,62% y 75% respectivamente.
CONCLUSIONES: El NEXUS CHEST tuvo una alta sensibilidad y baja
especificidad para los hallazgos radiográficos positivos. To assess the validity of the NEXUS Chest as decision
instrument for selectively chest radiographs in patients with chest trauma closed
at Regional Hospital of Trujillo during the period January 2010 to October 2014.
MATERIAL AND METHODS: We conducted an observational, analytical,
diagnostic test study, which evaluated 120 patients with blunt chest trauma,
distributed in groups I and II, with 96 and 24 patients respectively positive and
negative radiographic findings.
RESULTS: The overall mean age was 45.41 ± 18.57 years and 71.67% of the
study sample were male. The average NEXUS CHEST in group I and II were
2.16 ± 0.69 and 1.46 ± 0.93 respectively. Regarding the causes of trauma in both
groups predominated traffic accidents, followed by falls from height; and type of
injuries that predominated were hemothorax, pneumothorax followed by
multiple rib fractures. By associating intrathoracic injuries and NEXUS CHEST,
it was observed that 83.62% of patients with positive NEXUS CHEST had
intrathoracic injuries and 25% of patients who had a negative NEXUS CHEST
had intrathoracic lesions (p < 0.01); regarding the performance of the NEXUS
CHEST, to predict intrathoracic injuries, a cutoff ≥ 1, the sensitivity, specificity,
PPV and NPV were 98.98%, 13.64%, 83.62% and 75% respectively.
CONCLUSIONS: The NEXUS CHEST had high sensitivity and low specificity
for positive radiographic findings.
