Bicarbonato sérico como factor predictivo de muerte en pacientes con enfermedad pulmonar obstructiva crónica

OBJETIVO: Demostrar si el bicarbonato sérico es un factor predictivo de muerte en
pacientes con enfermedad pulmonar obstructiva.
MÉTODO: Estudio retrospectivo- pruebas diagnósticas, de 134 pacientes con
Enfermedad Pulmonar Obstructiva crónica en el Hospital Regional Docente de Trujillo
(HRDT) durante el periodo comprendido entre 2009- 2014, en quienes se determinó la
utilidad del valor de bicarbonato sérico como factor predictivo de mortalidad, a través
del cálculo de la razón de Sensibilidad (S), Especificidad (E), Prevalencia (P), valor
predictivo positivo y negativo (VPP, VPN), verosimilitud positiva y negativa(RVP,
RVN). Estuvo conformado por un grupo de pacientes que presentaron bicarbonato
sérico elevado y el otro grupo por aquellos con bicarbonato sérico normal.
RESULTADOS: La mortalidad global fue de 53.7% de los pacientes, siendo 61.2 y
7.5% en el grupo de bicarbonato sérico elevado y normal respectivamente. La
Sensibilidad (S) fue de 60.5, 4% (IC: 51.3. – 69.71), Especificidad (E) de 33.33% (IC:
6.14-60.52), Valor Predictivo Positivo (VPP) de 87.8% (IC: 80.11-95.5), Valor
Predictivo Negativo (VPN) de 9.62% (IC: 0.64-18.59), Razón de Verosimilitud Positiva
(RVP) de 0.91 (IC: 0.62-1.34), Razón de Verosimilitud Negativa (RVN) de 1.18 (IC:
0.56-2.51). La Prevalencia (P) encontrado fue de 88.81 (IC 83.09-94.52).
CONCLUSION: El bicarbonato sérico elevado no es un buen predictor de mortalidad
en pacientes con Enfermedad Pulmonar Obstructiva crónica. OBJECTIVE: Demonstrate if serum bicarbonate is a predictor of death in patients with
obstructive lung disease.
METHOD : A retrospective study - diagnostic tests, 134 patients with chronic obstructive
pulmonary disease at Regional Hospital of Trujillo ( HRDT ) during the period 2009- 2014,
in whom the utility of serum bicarbonate value was determined as predictor mortality, by
calculating the ratio of sensitivity ( S ) , specificity (E ) Prevalence ( P ) , positive and
negative predictive value ( PPV, NPV ) , positive likelihood ratio and negative ( RVP , RVN)
. It consisted of a group of patients who had elevated serum bicarbonate and the other group
for those with normal serum bicarbonate.
RESULTS: Overall mortality was 53.7 % of patients, with 61.2 and 7.5 % in the high normal
serum bicarbonate respectively. Sensitivity ( S ) was 60.5 , 4 % (CI : 51.3 - 69.71 . ) ,
Specificity (E ) of 33.33 % (CI : 6.14-60.52 ) , positive predictive value (PPV ) of 87.8 % (
CI 80.11-95.5 ) , negative predictive value ( NPV) of 9.62 % (CI : 0.64-18.59 ) , positive
likelihood ratio ( RVP ) of 0.91 (CI: 0.62-1.34 ) Negative Likelihood Ratio (RVN ) 1.18 (CI:
0.56- 2.51 ) . Prevalence (P) found was 88.81 (CI 83.09-94.52).
CONCLUSION: The elevated serum bicarbonate is not a good predictor of mortality in
patients with chronic obstructive pulmonary disease.
