Reporte de caso: sarcoma de kaposi gastrointestinal primario en un paciente con VIH/SIDA en hospital virgen de la puerta - Trujillo, Perú

Abstract

Paciente de 38 años procedente de Chocope, con historia de 2 años con síndrome constitucional asociado a diarreas, hace 1 año se le añade cándida orofaríngea y adenopatías cervicales e inguinales. En centro de salud de origen, se le diagnostica VIH en estadío SIDA, por encontrar prueba rápida positiva junto con clínica sugestiva ya mencionada. Por lo cual se le refiere a Hospital Virgen de la Puerta. Se le realizó endoscopía donde señala múltiples lesiones violáceas elevadas en esófago entre 3 y 8mm, entre 3 y 15mm en estómago y entre 5 a 5 mm en duodeno. Al examen microscópico la biopsia gástrica mostró mucosa gástrica con foco de hiperplasia foveolar, asociado a hemorragia instersticial e inflamación crónica con actividad leve. Por lo que se le solicita estudio de inmunohistoquímica encontrándose: HHSV-8 positivo, CD 31: positivo en vasos. Concluyendo como diagnóstico al Sarcoma de Kaposi gastrointestinal primario. Iniciándole Terapia Antirretroviral, para luego darle de alta para seguimiento por consultorio externo

Endoscopy was performed showing multiple raised violaceous lesions in the esophagus between 3 and 8mm, between 3 and 15mm in the stomach and between 5 and 5mm in the duodenum. On microscopic examination, the gastric biopsy showed gastric mucosa with a focus of foveolar hyperplasia, associated with interstitial hemorrhage and chronic inflammation with mild activity. Therefore, an immunohistochemical study is requested, finding: HHSV-8 positive, CD 31: positive in vessels. Concluding as a diagnosis primary gastrointestinal Kaposi's Sarcoma. Initiating Antiretroviral Therapy, and then discharged for outpatient follow-up.
