Hipoalbuminemia como factor asociado a peritonitis bacteriana espontánea en cirróticos del Hospital Lazarte

Objetivo: Determinar si existe asociación entre la hipoalbuminemia y la peritonitis
bacteriana espontánea (PBE) en pacientes con cirrosis hepática en el Hospital Víctor
Lazarte Echegaray en el período comprendido entre Enero 2010 y Junio 2016.
Material y Método: Se realizó un estudio observacional, analítico, transversal y
retrospectivo con un total de 160 pacientes atendidos en el servicio de Gastroenterología de
los cuales 79 si presentaron Peritonitis Bacteriana Espontánea y 81 pacientes, no. Para el análisis estadístico se aplicó la prueba de Chi-cuadrado ( 2 ) y la prueba del T student.
Resultados: Se determinó una fuerte asociación entre hipoalbuminemia y PBE (p<0.001), con un promedio de 2,20 ± 0,82g/dl para quienes presentaron la complicación versus 3,16 ±
0,88g/dl, para quienes no. Asimismo el Odds Ratio (OR) hallado de la asociación entre la hipoalbuminemia y PBE fue de 0.249 (Intervalo de confianza 95%: 0.122 – 0.510). La
edad promedio de ambos grupos de estudio fue de 68,80 ± 12,47años y 70,01 ± 10,26años
respectivamente (p>0.05). El género masculino tuvo una mayor incidencia en las dos
situaciones representado por el 50.63% y 61.73% (p>0.05). En cuanto a la etiología se
concluyó que la hepatitis alcohólica ocupo el primer lugar con un 22.78% y 22.22%
correspondiente a cada grupo (p>0.05). Además se encontró una alta significancia
estadística respecto al score de Child de Pugh C (p<0.001), estimando que 30.38% (con
PBE y el 3.7% (sin PBE) pertenecían a la clase C. De igual modo se analizó el recuento de
leucocitos asociándose fuertemente con la Peritonitis Bacteriana Espontánea (p<0.001) y
obteniendo 15040,38 ± 4771,11/L versus 6278,40 ± 4315,29/L como resultados de las dos
poblaciones (OR: 0.017; IC 95%: 0.005-0.061). En relación al LDH se obtuvo valores de
463,73 ± 206,45U/l y 300,16 ± 162,44U/l correspondiente a cada grupo (p<0.001) con OR:
0.184 (IC95%: 0.036-0.947). El TP alcanzo un valor de 21,49 ± 8,90s en pacientes con
PBE y, 18,62 ± 7,90s en pacientes sin PBE (p<0.05). Y de manera similar se halló que
también la Hb es un factor asociado a esta variable con un promedio de 10.18± 2,56mg/dl
y 10,97 ± 1,86mg/dl respectivamente (p<0.05) y con OR: 1.363 (IC95%:1.044-1.780).
Conclusión: Los pacientes cirróticos con hipoalbuminemia marcada asocian un mayor
riesgo de desarrollar Peritonitis Bacteriana Espontánea. Asimismo el LDH, recuento de
leucocitos y la hemoglobina fueron reconocidos como factores asociados a la patología. Objective: To determine the association between hypoalbuminemia and spontaneous
bacterial peritonitis (SBP) in patients with liver cirrhosis in Victor Lazarte Echegaray
Hospital in the period between January 2010 and June 2016
Methods: An observational, analytical, cross-sectional and retrospective study was
conducted with a total of 160 patients attending the Gastroenterology Service of which 79
presented Spontaneous Bacterial Peritonitis and 81 patients, no. For statistical analysis the
Chi-square test (x ^ 2) and T student test was applied.
Results: A strong association between albumin and PBE (p <0.001) was determined, with
an average of 2.20 ± 0,82g / dl for those who presented the complication versus 3.16 ±
0.88g / dl for those who do not. Also the Odds Ratio (OR) found the association between
hypoalbuminemia and PBE was 0.249 (95% confidence interval: 0122-0510). The average
age of both study groups was 68.80 ± 12,47years and 70.01 ± 10,26years respectively (p>
0.05). The male had a higher incidence in the two situations represented by 50.63% and
61.73% (p> 0.05). As for the etiology it was concluded that alcoholic hepatitis ranked first
with 22.78% and 22.22% for each group (p> 0.05). In addition a high statistical
significance with respect to score Child Pugh C (p <0.001) was found, estimating that
30.38% (with PBE and 3.7% (without PBE) belonged to class C. Likewise count was
analyzed leukocytes being associated with the variable in question (p <0.001) and getting
15040.38 ± 4771.11 /L versus 6278.40 ± 4315.29 / L as a result of the two populations
(OR: 0.017, 95% CI: 0.005 - 0061). In relation to the LDH, their values were 463, 73 ±
206,45U/l and 300, 16 ± 162,44U/l respectively to each group (p <0.001) with OR: 0.184
(IC95%: 0.036-0.947). The TP reached a value of 6278.40 ± 4315,0s in patients with PBE
and 18.62 ± 7,90s PBE in patients without (p <0.05). and similarly also found that Hb is an
associated with this variable with an average of 10.18 ± 2,56mg / dl and 10.97 ± 1,86mg/dl,
respectively (p <0.05) and OR: 1.363 (95% CI: 1,044-1,780).
Conclusion: Cirrhotic patients with hypoalbuminemia associated with an increased risk of
developing Spontaneous Bacterial Peritonitis. Also LDH, white blood cell count and
hemoglobin were recognized as factors associated with the complication.
