dc.description.abstract | OBJETIVO: Demostrar si el hematocrito y creatinina al ingreso, son factores
predictores de pancreatitis severa en pacientes del Hospital Regional Docente de
Trujillo en el periodo 2017-2020.
MATERIAL Y MÉTODO: Estudio de pruebas diagnósticas, retrospectivo, con
régimen de investigación libre. La población fueron un total de 129 pacientes con
diagnóstico de pancreatitis aguda en el Hospital Regional Docente de Trujillo
durante el periodo 2017-2020, que cumplieron con los criterios de selección. Se
recolectó el primer valor obtenido de hematocrito y creatinina dentro de las primeras
24 horas de estancia hospitalaria, calculándose la sensibilidad (S), especificidad (E),
valores predictivos positivos y negativos (VPP y VPN) de los puntos de corte
establecidos por medio de la curva ROC.
RESULTADOS: Se evaluaron 129 pacientes, de los cuales 29 tenían PA severa y
100 PA no severa. La severidad fue predominante en el género femenino (62% vs
38%), con una media de edad de 60+-19.6 y de etiología litiásica. El punto de corte
óptimo para determinar si el hematocrito al ingreso es factor predictor de severidad
fue de >= 45.3%, con una sensibilidad de 86.21%, especificidad de 88%, VPP de
67.57%; VPN de 87.6% e índice de validez 87.6%. Para la variable creatinina se
obtuvo un punto de corte de >= 0.9 mg/dl, con una sensibilidad de 93.1%,
especificidad de 87%, VPP de 67.5%, VPN de 97.75, e índice de validez 88.37%.
CONCLUSIONES: El hematocrito >= 45.3% y creatinina >= 0.9 mg/dl al ingreso,
dentro de las primeras 24 horas de estancia hospitalaria, son predictores tempranos
de pancreatitis aguda severa. | es_PE |

of severe pancreatitis in patients at the Hospital Regional Docente de Trujillo
between the period 2017-2020.
MATERIAL AND METHOD: Study of diagnostic tests, retrospective, analytic cross sectional, with free research regime. The population was a total of 129 patients
diagnosed with acute pancreatitis at the Hospital Regional Docente de Trujillo
between the period 2017-2020, who met the selection criteria. The first value for
hematocrit and creatinine was collected in the first 24 hours of admission in the
hospital, calculating sensitivity (S), specificity (E), positive and negative predictive
values (PPV and NPV) of the cut-off points established by ROC curve.
RESULTS: A total of 129 patients were evaluated, of whom 20 had severe AP and
100 had non-severe AP. The severity was predominant in the female gender (62%
vs 38%), with a mean age of 60+-19.6 and lithiasic etiology. The optimal cut-off point
to determine if hematocrit on admission is a predictor of severity was >= 45.3%, with
a sensitivity of 86.21%, specificity of 88%, PPV of 67.57%; NPV of 87.6% and validity
index 87.6%. For the creatinine variable, a cut-off point of >= 0.9 mg/dl was obtained,
with a sensitivity of 93.1%, specificity of 87%, PPV of 67.5%, NPV of 97.75, and a
validity index of 88.37%.
CONCLUSIONS: Hematocrit >= 45.3% and creatinine >= 0.9 mg/dl on admission,
in the first 24 hours of hospital stay, are early predictors of severe acute pancreatitis. | en_US |
