dc.description.abstract | Determinar si en pacientes que son sometidos a cirugía abdominal y pélvica mayor, el uso de la tromboprofilaxis extendida es más eficaz y segura que la tromboprofilaxis convencional a base de heparina de bajo peso molecular para disminuir la incidencia de eventos tromboembólicos. Métodos: Se realizó una revisión sistemática y metaanálisis. Se realizó una búsqueda de los estudios en PubMed, Web of Science, Scopus y EMBASE; se encontró un total de 2078 resultados, luego de realizar el tamizaje se obtuvieron 23 artículos que fueron revisados a texto completo, quedando 5 artículos, que fueron incluidos en la revisión sistemática. Los desenlaces principales y secundarios fueron medidos a través de un RR con un IC 95%. Resultados: La Incidencia de eventos tromboembólicos venosos fue menor en el grupo que recibió tromboprofilaxis extendida RR = 0,46; IC 95% [0,33-0,64]. En relación a los desenlaces secundarios, la incidencia de trombosis venosa profunda tuvo un efecto global en la reducción de la TVP del 54% a favor de la TP extendida, RR = 0,46; IC 95% [0,33-0,66] (p = 0,001). No se tuvo una diferencia significativa en la incidencia de embolia pulmonar en ambos tipos de tromboprofilaxis, así mismo, tampoco hubo una mayor incidencia de complicaciones hemorrágicas en ambos tipos de profilaxis. Conclusión: La tromboprofilaxis extendida redujo significativamente la aparición de eventos tromboembólicos venosos sin incrementar el riesgo de complicaciones hemorrágicas. | es_PE |
dc.description.abstract | To determine whether in patients undergoing major abdominal and pelvic surgery, the use of extended thromboprophylaxis is more effective and safer than conventional thromboprophylaxis based on low-molecular-weight heparin in reducing the incidence of thromboembolic events. Methods: A systematic review and meta-analysis was performed. A search for studies was carried out in PubMed, Web of Science, Scopus and EMBASE; a total of 2078 results were found. After screening, 23 articles were obtained and reviewed in full text, leaving 5 articles, which were included in the systematic review. Primary and secondary outcomes were measured by RR with 95% CI. Results: Incidence of venous thromboembolic events was lower in the group receiving extended thromboprophylaxis RR = 0.46, 95% CI [0.33-0.64]. Regarding secondary outcomes, the incidence of deep vein thrombosis had an overall effect on DVT reduction of 54% in favor of extended PT, RR = 0.46, 95% CI [0.33-0.66] (p = 0.001). There was no significant difference in the incidence of pulmonary embolism in both types of thromboprophylaxis, nor was there a higher incidence of bleeding complications in both types of prophylaxis. Conclusion: Extended thromboprophylaxis significantly reduced the occurrence of venous thromboembolic events without increasing the risk of hemorrhagic complications. | en_US |
