Efectividad del IBMP-10 y Apache II en la predicción de mortalidad en pacientes mayores de 18 años con neumonía asociada a ventilador

Objetivo: Determinar si el IBMP-10 es más efectivo que el APACHE II en la predicción de
mortalidad en pacientes mayores de 18 años con neumonía asociada a ventilador en el
departamento de emergencia y cuidados críticos del Hospital Belén de Trujillo (HBT).
Material y Métodos: Se realizó un estudio descriptivo observacional de corte transversal para
pruebas diagnósticas en 120 pacientes que cumplan con los criterios de selección. Se aplicó los
puntajes IBMP-10 y APACHE II. Se calculó la sensibilidad, especificidad y valores predictivos,
además del Área Bajo la Curva (ABC) ROC para comparar el IBMP-10 con la puntuación
APACHE II.
Resultados: La edad media fue 60.99 ±22.4 y el 51.7% (62) fueron de sexo femenino. La tasa
de mortalidad general fue de 36%. Para el punto de corte ≥2, la sensibilidad del IBMP-10 fue de
93.02%, la especificidad fue de 76.62%, el VPP fue de 68.97 % y el VPN fue de 95.16%; y para
el APACHE II, la sensibilidad fue de 83.72 %, la especificidad fue de 84.42%, el VPP fue de
75% y el VPN fue de 90.28%. El ABC ROC para el puntaje IBMP-10 para predecir mortalidad
fue de 0.885 (p<0.001) y de 0.871 (p <0.001) para el puntaje APACHE II.
Conclusiones: El IBMP-10 fue igual de efectivo que el APACHE II en la predicción de
mortalidad (p = 0.716). Objective: Define whether IBMP-10 is more effective than APACHE II predicting mortality in
patients older than 18 years with VAP in the emergency department and critical care of Belen
Hospital of Trujillo.
Methods: A cross-sectional observational descriptive study of diagnostic tests was executed for
patients who satisfy the selection criteria. APACHE II and the IBMP-10 scores were calculated.
Sensitivity, specificity and predictive values were calculated in addition the areas under the
receiver-operating characteristic curves (AUROCs) to compare the IBMP-10 with the APACHE
II score.
Results: The mean age was 60.99 ± 22.4 and 51.7% (62) were female. The overall mortality rate
was 36%. We chose the ≥2 as the cutoff point. For the IBMP-10 score, sensitivity was 93.02%,
specificity was 76.62%, PPV was 68.97% and NPV was 95.16%. And for the APACHE II score,
sensitivity was 83.72%, specificity was 84.42%, PPV was 75% and NPV was 90.28%. The
AUROCs for the IBMP-10 score to predict mortality was 0.885 (p <0.001) and 0.871 (p <0.001) for the APACHE II score.
Conclusion: The IBMP-10 was as effective as the APACHE II score in predicting mortality
(p-value = 0.716).
