Factores de riesgo para neumonía asociada a ventilación mecánica. Hospital Belén de Trujillo. 2011 - 2014

Abstract

La Neumonía asociada a Ventilación Mecánica, es una de las enfermedades más comunes en la Unidad de cuidados intensivos de adultos y se asocia a una alta tasa de morbilidad y mortalidad. Además es un proceso causal mulifactorial en el cual existen diferentes factores de riesgo que la desencadenan. El objetivo de la presente Investigación ha sido identificar qué factores de riesgo están relacionados con la Neumonía asociada a Ventilación Mecánica. Se realizó un estudio de casos y controles, donde se analizaron 42 casos, y 60 controles de pacientes hospitalizados en la Unidad de cuidados intensivos de Adultos y la Unidad de reanimación cardiopulmonar de Emergencia del Hospital Belén de Trujillo, durante el período de enero 2011 a diciembre 2014. Se observó que no hubo diferencias entre el sexo femenino y masculino en los pacientes que presentaron Neumonía asociada a Ventilador Mecánico. La edad promedio fue 56.2 años ± 23.64. Se encontró relación significativa entre las variables de estudio y la enfermedad, así tenemos: Ingreso de Noche (OR: 1.67, IC: 1.18 a 6.01, p: 0.0168), Intubación prolongada mayor de 7 días (OR: 1.82, IC: 1.2 a 6.6, p: 0.0157) y el Antecedente de Cirugía General (OR: 1.4, IC: 1.1 a 5.8, p: 0.0328), así mismo se encontró que el traslado fuera de la Unidad de Cuidados Intensivos fue altamente significativo (OR: 8, IC: 3,2 a 25.4, p: 0.000007). Se ha identificado que el ingreso de noche, la intubación prolongada mayor de 7 días, el traslado fuera de la Unidad de cuidados intensivos y el antecedente de Cirugía General, son factores de riesgo que aumentan la probabilidad de Neumonía asociada a Ventilación Mecánica.

The Pneumonia Associated with Mechanical Ventilation is one of the most common diseases in the adult intensive care Unit and is associated with a high rate of morbidity and mortality. It is also a multifactorial causal process in which there are different risk factors that trigger it. The objective of the present investigation is identify that risk factors are related to Pneumonia associated with Mechanical Ventilation. A case-control study was carried out, where 42 cases and 60 hospitalized controls were analyzed in the intensive care Unit of Adults and the cardiopulmonary resuscitation Unit of the Belen Hospital of Trujillo during the period From January 2011 to December 2014. It was observed that there were no differences between the female and the male sex in the patients who presented Mechanical Ventilator-associated Pneumonia. The mean age was 56.2 years ± 23.64. A significant relationship was found between the study variables and the disease, as follows: Night Admission (OR: 1.67, CI: 1.18 to 6.01, p: 0.0168), Prolonged intubation greater than 7 days (OR: 1.82, CI 1.2 a 6.6, p: 0.0157) and the Antecedent of General Surgery (OR: 1.4, CI: 1.1 to 5.8, p: 0.0328), it was also found that the transfer outside the Intensive Care Unit was highly significant (OR: 8, CI: 3.2 to 25.4, w: 0.000007). It has been identified that the night admission, prolonged intubation greater than 7 days, removal from the intensive care Unit and the antecedent of General Surgery are risk factors that increase the probability of Pneumonia associated with Mechanical Ventilation.
