Objective: To determine if hydronephrosis represents a predictor for complicated appendicitis in patients with acute appendicitis. Materials and methods: An observational, cross-sectional, analytical, and prospective study was carried out; for which 145 patients with a histopathological diagnosis of acute appendicitis were included, belonging to the General Surgery Service of Hospital Regional Docente de Trujillo from June to December 2022. These were classified into two groups according to pathology results: patients with complicated acute appendicitis and uncomplicated. The results of abdominal ultrasonography were reviewed, evaluating the presence or absence of right hydronephrosis. The data was entered into a statistical analysis using measures of central tendency and dispersion and chi square test, considering the value of P < 0.05 statistically significant. Results: The present study found that 95/145 (65.5%) patients had a histopathological diagnosis of complicated acute appendicitis and 50/145 (34.4%) patients had uncomplicated appendicitis. Of the patients with complicated acute appendicitis, 6.32% of them (6 patients) presented right hydronephrosis vs 2% (1 patient) in the other group, which did not represent a statistically significant association. Operative time was identified as an associated factor (p< 0.05) with complicated acute appendicitis, establishing an average of 71.34 ± 32.20 minutes vs 52.16 ± 21.16 minutes, for complicated and uncomplicated acute appendicitis respectively. Conclusion: The presence of right hydronephrosis is not a predictor of complicated acute appendicitis; however, its presence is clinically relevantes_PE
