Efectividad del colgajo de músculo glúteo mayor y colgajo cutáneo local en el tratamiento de úlceras sacras por presión

Abstract

OBJETIVO: Determinar la efectividad del colgajo de músculo glúteo mayor y colgajo cutáneo local en el tratamiento de úlceras sacras por presión en pacientes atendidos en el Hospital Víctor Lazarte Echegaray Nivel IV EsSalud de Trujillo durante el período comprendido entre Enero 1993 - Enero 2015 MATERIAL Y MÉTODO: Se realizó un estudio de cohorte, en donde se revisaron retrospectivamente los datos de 39 pacientes con úlceras sacras por presión tratados quirúrgicamente en el servicio de cirugía plástica. El proceso de selección fue realizado bajo los criterios de inclusión y exclusión, teniendo como total registrado a 47 pacientes pero por motivo de depuración y desaparecidos solo se consideró 39 historias clínica. Comparamos dos grupos: colgajo de músculo glúteo mayor (n =22) con técnica isla tunelizada y colgajo cutáneo local (n =17) con técnica rotacional. Un valor P < 0.05 se consideró estadísticamente significativo. RESULTADOS: No hubo diferencias estadísticamente significativas entre las variables sociodemográficas de los grupos con P > 0.05. El riego relativo del grupo colgajo glúteo mayor fue de18.2 % y del colgajo cutáneo local fue de 41.2 %. El grupo de colgajo de glúteo mayor tuvo una incidencia estadísticamente significativa más alta de 81.8 % que el grupo de colgajo cutáneo local de 58.8 %. Sin embargo, las complicaciones postoperatorias fueron mayores para el grupo cutáneo local con un RR de 2,26 por cada un fallido con CMGM. IC 95%. CONCLUSIONES: La técnica quirúrgica de colgajo de músculo glúteo mayor es segura y no conduce a un aumento de las complicaciones. Sin embargo, la técnica se dificulta por la gran pérdida sanguínea que es difícil de controlar, a pesar de eso, la tasa de efectividad es mayor que la del colgajo cutáneo local, pero no es estadísticamente significativo.

OBJECTIVE: To determine the effectiveness of the gluteus maximus muscle flap and local cutaneous flap in the treatment of sacral ulcers due to pressure in patients treated at the Víctor Lazarte Echegaray Hospital Level IV EsSalud of Trujillo during the period between January 1993 and January 2015. MATERIAL AND METHOD: A cohort study was conducted, in which data from 39 patients with sacral ulcers due to pressure treated surgically in the plastic surgery service were retrospectively reviewed. The selection process was carried out under the inclusion and exclusion criteria, having 47 patients as a total, but due to purification and disappearance only 39 clinical histories were considered. We compared two groups: gluteus maximus muscle flap (n = 22) with tunelized island technique and local cutaneous flap (n = 17) with rotational technique. The relative irrigation of the gluteus flap greater group 18.2% and the local cutaneous flap 41.2%. The gluteus maximus flap group had a statistically significant higher incidence of 81.8% than the local cutaneous flap group of 58.8%.
