Efectividad del índice PSA libre versus PSA total en el diagnóstico de cáncer de próstata

Abstract

OBJETIVO: Determinar si el Índice PSA libre es más efectivo que el PSA total en el diagnóstico de cáncer de próstata en pacientes con sospecha de cáncer de próstata que acuden a consultorio externo de urología del Hospital Víctor Lazarte Echegaray en el periodo comprendido entre enero 2 014- enero 2 018. MATERIAL Y MÉTODO: Se realizó un estudio de pruebas diagnósticas; el proceso de selección fue realizado bajo los criterios de inclusión y exclusión, teniendo como total de registrados a 850 pacientes, por motivos de depuración sólo se consideraron 190 que fueron evaluados con nuestra hoja de recolección de datos. RESULTADOS: De los 190 pacientes, 99 presentaron cáncer y 91 no presentaron, la edad promedio en años fue de 70,82+-5,9 y 64,75+-6,84 respectivamente. Estadísticamente se obtuvo un punto de corte para Índice PSA libre de 0,16 con un registro de área bajo la curva ROC de 0,996; IC 95% y significancia estadística (p<0,001); demostrando una sensibilidad de 98,99%, especificidad de 94,51%, valor predictivo positivo de 95,15% y valor predictivo negativo de 98,85%. Se obtuvo una exactitud diagnóstica para Índice PSA libre de 96,84% frente a 54,21% de PSA total con punto de corte de 4ng/ml. CONCLUSIONES: La prueba diagnóstica Índice PSA libre presenta mayor efectividad que el PSA total en el diagnóstico de cáncer de próstata, y que, a un punto de corte de 0,16, detecta la enfermedad en un 98,99%, y demuestra una mejora de la especificidad del PSA total para evitar las biopsias innecesarias, especialmente cuando los valores se encuentran en la zona gris.

OBJECTIVE: To determine if the free PSA Index is more effective than the total PSA in the diagnosis of prostate cancer in patients with suspected prostate cancer who come to an outpatient clinic of urology of the Víctor Lazarte Echegaray Hospital in the period between January 2 014- January 2 018. MATERIAL AND METHODS: A study of diagnostic test was carried out; the selection process was carried out under inclusion and exclusion criteria, having 850 patients as a whole, but for purification reasons, only 190 were considered that were evaluated with our data collection sheet. RESULTS: Of the 190 patients, 99 had cancer and 91 did not present, the average age in years was 70.82+-5.9 and 64.75+-6.84 respectively. Statistically, a cut-off point was obtained for a free PSA Index of 0.16 with an area record under the ROC curve of 0.996; IC 95% and statistical significance (p<0.001); showing a sensitivity of 98.99%, specificity of 94.51%, positive predictive value of 95.15% and negative predictive value of 98.85%. Diagnostic accuracy was obtained for free PSA Index of 96.84% versus 54.21% of total PSA with a cut-off of 4 ng/ml. CONCLUSIONS: The free PSA Index test is more effective than total PSA in the diagnosis of prostate cancer, and at a cut-off 0.16, it detects the disease in 98.99%, and demonstrates an improvement in the specificity of total PSA to avoid unnecessary biopsies, especially when the values are in the gray area.
