To determine the relationship between oral health-related quality of life and early childhood caries in preschoolers from different schools in the city of Trujillo in the year 2023. Materials and Methods: A cross-sectional and correlational study was conducted with 302 preschoolers aged 3 to 5 years from various schools. The ceo-d index was used to detect early childhood caries (ECC), and the Early Childhood Oral Health Impact Scale (ECOHIS) questionnaire was administered to their family members to assess perceptions of quality of life, considering factors such as sex, type of school, and the age of the preschooler and caregiver. Mann- Whitney U tests, Kruskal-Wallis tests, and a Generalized Linear Model (GLM) with Tweedie regression were used to estimate crude and adjusted prevalence ratios (PR and APR). Results: The prevalence of early childhood caries was 24.5%, and 34.4% for severe caries. The average ECOHIS score was 3.20 ± 4.55. Significant associations were found between oral health-related quality of life and early childhood caries (p < 0.001) and type of school (p < 0.05). Inferential analysis showed that early childhood caries and severe early childhood caries had 5 and 9.5 times higher probabilities, respectively, of negatively impacting oral health-related quality of life compared to preschoolers without caries. Conclusion: There is a statistically significant relationship between oral health-related quality of life and early childhood caries in preschoolers from different schools in the city of Trujillo in the year 2023.es_PE
