Influencia de la edad gestacional en los niveles de hormonas tiroideas según tamizaje neonatal en Hospital II Essalud Huaraz enero 2014 - diciembre 2018

Abstract

Objetivo: Determinar si la edad gestacional influye sobre los niveles de hormonas tiroideas detectados por tamizaje neonatal. Material y Métodos: Mediante un diseño transversal, analítico, retrospectivo en el Hospital II de Essalud de Huaraz, se revisaron los resultados de TSH de 1226 recién nacidos además de tomar en cuenta datos de peso al nacer, edad gestacional y tipo de parto obtenidos por tamizaje neonatal de nacidos vivos entre el 2014 y el 2018. Resultados: Se estudiaron 1226 recién nacidos (RN) de los cuales 1156 fueron a término y 70 fueron pre término. No se encontró recién nacidos pos término en el presente estudio. La incidencia anual de hipotiroidismo congénito (HC) fue de 0.82 casos x 1000 RN vivos o 1 caso de HC por cada 1220 RN vivos. El nivel promedio de TSH de los RN prematuros fue de 2.19 ± 1.65 mUI/L, mientras que en los RN a término fue de 2.50 ± 3.66 mUI/L, no encontrando diferencias significativas entre los niveles de TSH de RN prematuros y a término por U de Mann Withney (p=0.4088). Por otro lado, el análisis de regresión de Spearman para datos no normales entre la edad gestacional y los niveles de TSH encontrados por tamizaje neonatal en la población de estudio fue de Rho=0.006 y p=0.8185. No existe influencia significativa del sexo, peso al nacer y tipo de parto en la relación entre edad gestacional y niveles de hormonas tiroideas. Conclusiones: No se encontró correlación entre la edad gestacional y los niveles de TSH en la población de estudio. Por otro lado, los niveles de TSH entre prematuros y nacidos a término no tuvieron diferencias significativas.
