Valor de los índices de neutrófilo/linfocito y plaqueta/linfocito como predictores de neumonía en menores de cinco años

Abstract

El objetivo fue determinar si los índices neutrófilo/linfocito y plaqueta/linfocito pueden emplearse como predictores de neumonía en pacientes menores de cinco años. MATERIAL Y MÉTODO: Se realizó un estudio observacional, analítico de pruebas diagnósticas, durante el período comprendido entre el 01 de enero del 2018 al 31 de diciembre del 2019 en el Hospital Belén de Trujillo; se evaluaron 110 pacientes con neumonía, de los cuales 82 con radiografía de tórax compatible y 28 con radiografía no compatible. RESULTADOS: La edad en los grupos con y sin radiografía de tórax compatible con neumonía fueron 35.44 ± 18.70 y 34.60 ± 19.73 meses respectivamente (p>0.05); la proporción de varones en cada grupo fueron 53.7% y 53.6% respectivamente (p>0.05) y procedencia urbana de 29.2% en el primer grupo y 25% en el segundo grupo y procedencia rural de 70.8% en el primer grupo y 75% en el segundo grupo. Los valores del recuento de leucocitos, neutrófilos, plaquetas y linfocitos absolutos, INL e IPL en los grupos con y sin radiografía compatible fueron, los leucocitos 13.43 ± 5.10 y 10.14 ± 2.53 (p<0.001); neutrófilos 10.62 ± 4.81 y 6.72 ± 2.74 (p<0.001); plaquetas 309.76 ± 74.41 y 273.26 ± 76.13, linfocitos 2.05 ± 0.83 y 2.74 ± 0.75 (p<0.001); INL 6.07 ± 3.77 y 2.59 ± 1.11 (p<0.001) e IPL 174.10 ± 76.37 y 103 ± 29,69 (p<0.001) respectivamente. Tomando como punto de corte el valor de INL de CONCLUSIONES: Sugerimos que el INL e IPL constituyen marcadores factores predictores de neumonía en menores de cinco años y pueden emplearse como herramienta diagnóstica complementaria.

The objetive was to determine if the neutrophil-to-lymphocyte and platelet-to-lymphocyte ratios are markers of inflammation useful in predicting the diagnosis of pneumonia in patients younger than five years old. MATERIAL AND METHOD: An observational, analytical study of diagnostic test was conducted during the period from January 1 st , 2018 to December 31 , 2019 in Bethlehem Hospital of Trujillo; we evaluated 110 diagnosed with pneumonia, from which 82 had compatible findings in the chest radiography and 28 without compatible findings. RESULTS: The age in the groups with and without compatible findings with pneumonia on the chest radiography were 35.44 ± 18.70 and 34.60 ± 19.73 months respectively (p>0.05); the proportion of males in each group 53.7% and 53.6% respectively (p>0.05) and their urban procedence of 29.2% and 25% for each group; and rural procedence of 70.8% and 75% for each group. Taking as a cut-off point the NLR value CONCLUSIONS: We suggest that NLR and PLR are useful inflammatory markers to predict pneumonia in patients younger than five years old and can be used as a complement diagnostic tool. st
