La enfermedad de Ménétrier (EM) es una gastropatía hipertrófica muy
infrecuente de etiología desconocida, relacionada a hipoproteinemia,
hipoclorhidria y además cursa con engrosamiento de pliegues mucosos del
fondo y del cuerpo gástrico.
La gastroscopía con biopsias convencionales no siempre diagnóstica, por lo
tanto; ante una suposición se debe realizar macrobiopsia endoscópica con asa
de polipectomía que abarque todo el grosor de la mucosa dañada. En muchas
ocasiones un tránsito esófago gastroduodenal (TEGD) orienta el diagnóstico.
Se presenta el reporte clínico de una joven de 26 años de edad, donde fue
atendida en el Departamento de Cirugía del Hospital Docente Belén de
Lambayeque, la cual fue operada por cirugía electiva con el diagnóstico
preoperatorio de Litiasis Vesicular, que ingresa por presentar dolor abdominal
tipo cólico, pérdida de peso y vómitos, con ecografía abdominal sugerente a
cálculos vesiculares. La paciente persiste el mismo cuadro clínico posterior a la
cirugía, por lo que se realiza un tránsito esófago gastroduodenal seriado
(estático) y un estudio dinámico, donde se sospecha radiológicamente la
enfermedad de Ménétrier; se realiza una endoscopía reforzando la sospecha, se
toma una biopsia y el examen anatomopatológico corroboró el diagnóstico Ménétrier's disease (MS) is a very infrequent hypertrophic gastropathy of
unknown etiology, related to hypoproteinemia, hypochlorhydria and also causes
thickening of the mucosal folds of the gastric fundus and body.
Gastroscopy with conventional biopsies is not always diagnostic, therefore; when
faced with an assumption must be made by performing endoscopic macrobiopsy
with polypectomy loop that covers the full-thickness of the damaged mucosa. On
many occasions, an esophageal-gastroduodenal transit (EGDT) can orients the
diagnosis.
We present the clinical report of a 26 year-old young woman, who was treated at
the Surgery Department of the Hospital Docente Belén of Lambayeque. She was
operated by elective surgery with the pre-surgery diagnosis of Vesicular Lithiasis,
who was admitted for colicky type abdominal pain, weight loss and vomiting, with
abdominal ultrasound suggestive of vesicular stones. The patient persists the
same clinical picture after surgery, so a serial esophago-gastroduodenal transit
(static) and a dynamic study are performed, where Ménétrier's disease is
suspected radiologically; an endoscopy is performed reinforcing the suspicion, a
biopsy is taken and the anatomopathological examination corroborates the
diagnosis.
