Subdiagnóstico de mononucleosis infecciosa en pediatría

Abstract

Objetivos: El estudio tiene como principal objetivo estimar el subdiagnóstico de mononuclesis infecciosa en el Perú. Además, revisar los aspectos básicos de la enfermedad, su etiología y diagnóstico adecuado y recopilar casos donde el subdiagnóstico de mononucleosis infecciosa trajo complicaciones en la salud del paciente. Procedimientos: Se revisó los artículos publicados en los últimos 10 años en las bases de datos Pubmed, ClinicalKey y Google académico encontrados usando las palabras clave: “infectious mononucleosis”, “diagnostic”, “case report”, “complications”. Los artículos fueron seleccionados según criterio del equipo investigador. Principales hallazgos: La mononucleosis infecciosa es un síndrome clínico que es causado por múltiples agentes, principalmente el virus Epstein Barr, y que está caracterizado por fiebre, linfadenopatía y faringitis asociado a células mononucleares atípicas y linfocitosis. En el Perú puede que exista un subdiagnóstico de esta patología según lo reportado por las últimas publicaciones del Instituto Nacional de Salud en contraste con la realidad reportada en estudios de otros países. Este síndrome, generalmente benigno, puede traer consigo serios problemas para la salud del paciente, que van desde los casos fulminantes hasta complicaciones graves. Conclusiones: Se deduce según lo revisado que la mononucleosis infecciosa es un síndrome subdiagnósticado en el Perú. Las complicaciones recopiladas muestran que la mononucleosis infecciosa no siempre es una patología benigna.

Objectives: The main objective of the study is to estimate the underdiagnosis of infectious mononucleosis in Peru. In addition, review the basic aspects of the disease, its etiology and proper diagnosis, and compile cases where the underdiagnosis of infectious mononucleosis brought complications to the patient's health. Procedures: The articles published in the last 10 years in the Pubmed, ClinicalKey and Google academic databases found were reviewed using the keywords: “infectious mononucleosis”, “diagnostic”, “case report”, “complications”. The articles were selected according to the criteria of the research team. Main findings: Infectious mononucleosis is a clinical syndrome that is caused by multiple agents, mainly the Epstein Barr virus, and that is characterized by fever, lymphadenopathy and pharyngitis associated with atypical mononuclear cells and lymphocytosis. In Peru there may be an underdiagnosis of this pathology as reported by the latest publications of the National Institute of Health in contrast to the reality reported in studies from other countries. This syndrome, generally benign, can bring with it serious problems for the patient's health, ranging from fulminant cases to serious complications. Conclusions: According to the review, it is deduced that infectious mononucleosis is an underdiagnosed syndrome in Peru. Complications collected show that infectious mononucleosis is not always a benign pathology.
