dc.description.abstract | Determinar si la longitud cervical es un factor predictor de parto
pretérmino en gestantes con amenaza de parto pretérmino.
Material y métodos: Se realizó una investigación de pruebas
diagnósticas que abarcó a 150 gestantes que tenían amenaza de parto
pretérmino. Se dividió dos grupos: uno con parto pretérmino y el otro grupo sin
ello. Se inició analizando el punto de corte de ≤ 25 mm, posteriormente se calculó
la curva ROC para encontrar la propuesta de un nuevo punto de corte de longitud
cervical corta para diagnóstico de parto pretérmino a través de la ecografía
transvaginal, a partir de lo anterior se calculó en ambos puntos de corte la
sensibilidad, especificidad, el valor predictivo positivo y negativo, índice de
validez y la razón de verosimilitud positiva y negativa.
Resultados: Se obtuvieron 74,74% y 87,27%, como resultados para
sensibilidad y especificidad de una longitud cervical corta predictora de parto
prematuro respectivamente. El valor predictivo positivo y el valor predictivo
negativo de la longitud cervical corta fueron 91,03 y 66,67%, respectivamente.
En la curva de ROC, el nuevo punto es ≤ 27, 95 mm, cuya sensibilidad,
especificidad y valores predictivos positivo y negativo fueron 77,89%, 87,27%,
91,36% y 69,57% respectivamente. La exactitud pronóstica para este nuevo
punto de corte a modo de predictor de parto pretérmino fue 84%, siendo una
buena precisión diagnóstica. No se evidenciaron mayores diferencias
importantes en relación a las variables: nivel educativo, obesidad, talla baja, ni
estado civil entre los pacientes con o sin parto pretérmino.
Conclusión: La longitud cervical es factor predictor de parto pretérmino

dc.description.abstract | To determine if cervical length is a predictor of preterm delivery
in pregnant women with threatened preterm delivery.
Material and methods: An investigation of diagnostic tests was carried
out that included 150 pregnant women who had threatened preterm delivery. Two
groups were divided: one with preterm delivery and the other group without it. It
was started by analyzing the cut-off point of ≤ 25 mm, then the ROC curve was
calculated to find the proposal for a new cut-off point of short cervical length for
the diagnosis of preterm delivery through transvaginal ultrasound. At both cut-off
points, sensitivity, specificity, positive and negative predictive value, validity
index, and positive and negative likelihood ratio were calculated.
Results: 74.74% and 87.27% were obtained, as results for sensitivity and
specificity of a short cervical length predictor of preterm birth, respectively. The
positive predictive value and the negative predictive value of short cervical length
were 91.03 and 66.67%, respectively. In the ROC curve, the new point is ≤ 27.95
mm, whose sensitivity, specificity and positive and negative predictive values
were 77.89%, 87.27%, 91.36% and 69.57% respectively. The prognostic
accuracy for this new cut-off point as a predictor of preterm delivery was 84%,
being a good diagnostic precision. No major differences were found in relation to
the variables: educational level, obesity, short stature, or marital status among
patients with or without preterm delivery.
Conclusion: Cervical length is a predictor of preterm delivery in pregnant
