Morbimortalidad y tiempos críticos de trombólisis en eventos cerebrovasculares isquémicos. Emergencia del hospital Lazarte

El evento cerebral vascular (ECV) isquémico es la principal causa de incapacidad y
segunda causa de mortalidad en el mundo, tiene estrecha relación su aparición y tórpida
evolución con la existencia de factores de riesgo, la mortalidad a 30 días para ECV
isquémico se encuentra entre 10 y 17%. Las guías para tratamiento recomiendan el uso
de activador tisular del plasminógeno recombinante en las primeras 3 h del inicio de los
síntomas. En el presente trabajo observacional, longitudinal, descriptivo, analítico
estudiamos la morbimortalidad y los tiempos críticos para trombólisis en pacientes con
diagnóstico de ECV isquemico entre enero 2013-diciembre 2015. Encontramos que la
enfermedad tuvo mayor incidencia en la 7ma. y 8va. décadas de la vida, los factores de
riesgo más frecuentes fueron la hipertensión arterial y tabaquismo, los protocolos de
actuación no fueron cumplidos debidamente por múltiples causas; se observaron
retrasos en los tiempos críticos de arribo al hospital, evaluación clínico-tomográfica,
valoración por neurólogo y trombólisis. De 150 pacientes con EVC isquémico, 12 (8%)
completaron su evaluación antes de 180 minutos. Sólo 6 pacientes (4%) recibieron
trombólisis. En ambos grupos (menor de 180 minutos y mayor de 180 minutos), el
principal tiempo de retardo fue la valoración por el neurólogo. Los días de estancia
hospitalaria fueron menores en aquellos pacientes con un arribo y valoración
hospitalaria más temprana, siendo aún mayor el beneficio en aquellos pacientes que
recibieron trombólisis donde la sobrevida fué del 100% con recuperación completa y sin secuelas. The cerebral vascular event (ECV) ischemic is the leading cause of disability and
