OBJETIVO: Establecer los factores asociados a infección de sitio operatorio (ISO) en
artroplastia total de rodilla (ATR), en el Hospital Víctor Lazarte Echegaray, EsSalud en el
período 2005-2014. Determinar la incidencia acumulada de ISO en artroplastia total de
rodilla. Construir un modelo de predicción para ISO en artroplastia total de rodilla.
MATERIAL Y MÉTODO: Se realizó un estudio observacional, retrospectivo y
transversal en el período de enero del 2005 a diciembre del 2014, Hospital Víctor Lazarte
Echegaray, EsSalud. Se estudiaron los expedientes de los 262 pacientes intervenidos de
cirugía de prótesis total de rodilla, sólo 202 cumplieron con los criterios de inclusión. Se
almacenó la información en Excel y se pasó a los programas estadísticos R y SPSS 23.0,
procediendo a realizar análisis de frecuencias, correlación y análisis multivariado de
regresión logística. Se realizó un modelo predictor a través de la curva ROC.
RESULTADOS: De los 202 pacientes estudiados, se encontraron 17 casos de ISO (8,4%).
Se encontró que dentro de las características generales, el IMC fue la única variable con
significancia estadística, con una media en el grupo ISO de 30,09 ± 4,45 kg/m2 comparado
con la media 27,57 ± 3,40 kg/m2de los pacientes sin ISO, obteniéndose un valor de p<
0,01. El IMC promedio fue 28,06 ± 3,91 kg/m2. En los antecedentes mórbidos,
Desnutrición se encontró como factor asociado a ISO. Esta variable obtuvo un OR de
30,38 (IC 95% de 5,95 – 155,06). De los factores perioperatorios, el uso de Hemovac
presentó significancia estadística, con un OR de 2,96 (IC 95% de 0,95 – 9,23). La Curva
ROC presentó un área bajo la curva de 0,7836.
CONCLUSIONES: La incidencia de ISO es 8,4% en ATR. El IMC, la Desnutrición y el
uso de Hemovac son factores asociados a ISO en ATR. De éstas, sólo 2 variables
Desnutrición y uso de Hemovac, tienen una potencia de predicción de 78,36% para ISO en
ATR. NIVEL DE EVIDENCIA: 2B OBJECTIVE: To establish the factors associated with surgical site infection (SSI) in total
knee arthroplasty (TKA) at the Hospital Victor Lazarte Echegaray, EsSalud in 2005-2014.
To determine the incidence of SSI in total knee arthroplasty. Build a prediction model for
SSI in total knee arthroplasty.
MATERIALS AND METHODS: An observational, retrospective and transversal study
was conducted during the period January 2005 to December 2014, Victor Lazarte
Echegaray Hospital, EsSalud. The records of 262 patients who underwent surgery for total
knee replacement, of which only 202 met the inclusion criteria were studied. Informat ion
was stored in Excel and transferred to the R and SPSS 23.0 statistical software, proceeding
to perform frequency analysis, correlation and multivariate logistic regression analysis. A
predictor model through a ROC curve was performed.
RESULTS: Of the 202 pat ients studied, 17 cases of SSI (8.4%) were found. It was found
that within the general characteristics, BMI was the only variable was statistically
significant BMI, finding an average of 30,09 ± 4,45 kg/m2 in patients who developed SSI,
unlike an average of 27,57 ± 3,40 kg/m2 in patients without SSI; with a value of p <0.01.
The overall average was 28,06 ± 3,91 kg / m2. In the morbid history, Desnutrition was the
only associated factor. It had an OR of 30,38 (95% CI 5,95 – 155,06). Perioperative factors,
Hemovac use showed statistical significance, with an OR of 2,96 (95% CI 0,95 – 9,23).

CONCLUSIONS: The cumulative incidence of SSI is 8.4% in TKA. BMI, Desnutrit ion
and Hemovac use are SSI factors associated with total knee arthroplasty. Only 2 of them,
Desnutrit ion and Hemvac use, have a predictive power of 78.36% for SSI in TKA.
