Falla en la extubacion de ventilación mecánico en neonatos prematuros y asociación con el uso profiláctico de aminofilina

Determinar si existe asociación entre el uso profiláctico de la aminofilina
con la falla en la extubación del ventilador mecánico en neonatos menores de 34
semanas de gestación atendidos en el Departamento de Neonatología del Hospital Belén
de Trujillo durante el periodo de estudio julio 2015 – julio 2016
MATERIAL Y MÉTODOS: Estudio analítico, observacional, de cohorte histórico. El
tamaño de la muestra estuvo constituido por 74 neonatos prematuros en ventilación
mecánica según criterios de inclusión y exclusión. La información se obtuvo de las ficha
de recolección de datos de las historias clínicas del archivo del Hospital Belén
de Trujillo del periodo 2015-2016. Para el análisis de los resultados se utilizó chi
cuadrado y se halló el Riesgo Relativo (RR).
RESULTADOS: La eficacia del uso de aminofilina profiláctica se establece con un
81% de no falla en la extubación del ventilador mecánico. El porcentaje de neonatos
que no recibieron profilaxis con aminofilina y que hicieron falla en la extubación del
ventilador mecánico fue de 65%. Se analiza la relación que existe entre el uso
profiláctico de aminofilina y la falla en la extubación de ventilador mecánico,
obteniendo un RR de 3.4; es decir los neonatos prematuros < de 34 semanas de edad
gestacional sin uso profiláctico de aminofilina tiene 3.4 veces más probabilidad, le
confiere riesgo, de falla en la extubación..
CONCLUSIÓN: -En la presente investigación se ha determinado que el uso
profiláctico de aminofilina tiene una alta asociación con la falla en la extubación de
ventilador mecánico en neonatos prematuros, disminuye su frecuencia. To determine if association exist between the prophylactic use of the
aminophylline with the flaw in the extubation of the mechanical ventilator in neonates
younger than 34 weeks of gestation treated at the Department of Neonatology of
Hospital Belén of Trujillo during the study period July 2015 - July 2016.
MATERIAL AND METHODS: Analytical, observational, historical cohort study. The
sample size consisted of 74 premature neonates in mechanical ventilation according to
inclusion and exclusion criteria. The information was obtained of the card of
compilation of information of the clinical histories of the Hospital Belén of Trujillo of
the period 2015-2016. For the analysis of the results square was used x

the Relative Risk (RR).
RESULTS: The efficacy of use prophylactic aminophylline is established by a 81% of
non-failure in the extubation. The number of infants who did not receive aminophylline
prophylaxis and who failed to extubation the mechanical ventilator from those who did
not fail were similar. The percentage of neonates who did not receive the profile with
aminophylline and who failed to extubate the mechanical ventilator was 65%. The
relationship between the prophylactic use of aminophylline and the failure in
mechanical ventilator extubation is analyzed, obtaining an RR of 3.4; Therefore,
preterm infants <34 weeks of gestational age without prophylactic use of aminophylline
are 3.4 times more likely to pose a risk of failure of extubation.
CONCLUSION: In the present investigation it has been determined that the
prophylactic use of aminophylline has a high association with the failure of mechanical
ventilator extubation in preterm infants, being of a protective nature for this.
