Episiotomía mediolateral como factor protector de desgarro perineal en parturientas del Hospital Regional Docente de Trujillo

Objetivo: Determinar si la episiotomía mediolateral es factor protector para Desgarros
Perineales en parturientas del Hospital Regional Docente de Trujillo en el periodo entre enero
y diciembre del 2017.
Material y métodos: Se realizó un estudio de cohortes retrospectivo en 148 parturientas del
servicio de Obstetricia del HRDT en el 2017, de las cuales 74 pertenecieron al grupo de
estudio (con episiotomía medio lateral) y 74 al grupo control (sin episiotomía). Se realizó
medidas de tendencia central y de dispersión para variables cuantitativas (Edad, Talla, IMC,
Estancia, peso del RN). Para el análisis estadístico para comparar las variables episiotomía
mediolateral y desgarro perineal, se aplicó la prueba de Chi-cuadrado, seguido del Riesgo
Relativo para estimación de riesgo.
Resultados: El promedio de edad de las parturientas fue de 28,3 (±5.2). Asimismo la talla
fue de 1,54 cm (±0.04) y el IMC de 24,9 (±3.3). La estancia hospitalaria promedio fue de 1,7
días (±0.9) y el peso de los RN fue de 3322 grs (±360). El 27% procedían de La Esperanza,
el 28,4 % contaba con secundaria completa y el 25% una paragravidez de 2002. En cuanto a
la analítica, no se encontró asociación entre la episiotomía mediolateral y la protección de
desgarro perineal (p˃0,05). Sin embargo, si hubo asociación entre la condición de gran
multípara y desgarro perineal (p=0,004; RR=0,202 [0,03-0.639], haciendo que dicha
condición sea considerada un factor protector para desgarros perineales.
Conclusión: La episiotomía mediolateral no constituye un factor protector de desgarro
perineal. Objective: To determine if the mediolateral episiotomy is a protective factor for Perineal
tears in parturients of the Hospital Regional Docente of Trujillo in the period between
January and December 2017.
Material and methods: A retrospective cohort study was performed on 148 parturients in
the obstetrics service of the HRDT in 2017, of which 74 belonged to the study group (with
mediolateral episiotomy) and 74 to the control group (without episiotomy). We performed
central tendency and dispersion measures for quantitative variables (Age, Size, BMI, Stay,
RN weight). For the statistical analysis to compare the variables mediolateral episiotomy and
perineal tear, the Chi-square test was applied, followed by the Relative Risk for risk
estimation.
Results: The mean age of the parturients was 28.3 (± 5.2). Size was also 1.54 cm (± 0.04)
and BMI of 24.9 (± 3.3). The mean hospital stay was 1.7 days (± 0.9) and the weight of the
RN was 3322 grs (± 360). 27% came from La Esperanza, 28.4% had complete high school
and 25% had a obstetric history of 2002. Regarding the analytical, no association was found
between the mediolateral episiotomy and the perineal tear protection (p˃0,05). However,
there was an association between the great multiparous women condition and the perineal
tear (p=0.004; RR = 0.202 [0.03-0.639]), so this condition was considered a protective factor
for perineal tears.
Conclusion: Mediolateral episiotomy is not a protective factor of perineal tear.
