E: This study aims to determine the risk of prematurity due to exposure to preeclampsia during pregnancy in pregnant women treated in the gynecological-obstetrics emergency service of a hospital in Piura. MATERIAL AND METHODS: An observational, analytical, retrospective cohort study was conducted. 91 patients with preeclampsia (exposed cohort) and 91 patients without preeclampsia (unexposed cohort) were included. A probabilistic sampling was used; in addition, inclusion and exclusion criteria were applied to ensure the homogeneity of the cohorts. A univariate analysis was performed. RESULTS: Pregnant women with preeclampsia were found to have a tendency to have higher baseline characteristics such as age and anthropometric measurements, such as pre-pregnancy weight and BMI, compared to the nonexposed cohort (33rd vs 31st, 67 kg vs 60 kg, 27.26 vs 25.39) respectively. In addition, the exposed cohort had a lower gestational age at delivery (36 weeks versus 38 weeks in pregnant women without preeclampsia). Pregnant women with preeclampsia had a higher incidence of premature births (73.6%) compared to those who did not have this complication (26.4%). Likewise, the severity of preeclampsia influenced the tendency to prematurity, with 40.3% in mild cases and (33.3%) in severe cases. A moderate but positive correlation (r= 0.4208) was found between the level of proteinuria and the degree of severity of prematurity, such that, at higher levels of proteinuria, the severity of prematurity was greater. CONCLUSIONS: Pregnant women diagnosed with preeclampsia have a relative risk of 5.212 of presenting prematurity compared to those without a diagnosis of preeclampsiaes_PE
